While caring for aging patients, founder, Darin Patmon, MD, saw patients repeatedly harmed by gaps in the system – not because of lack of effort, but because providers simply didn’t have access to the right data at the right time. Those blind spots led to poor outcomes, high-risk complications, and preventable losses of life.
For Darin, the inequities were impossible to ignore. Systemic blind spots in care transitions disproportionately harmed rural clinics, minority communities, and underserved patients. At the same time, providers were overwhelmed and overburdened – missing revenue opportunities and struggling under compliance pressures.
He knew there had to be a better way. That conviction brought together physicians and technologists determined to design a platform that made care management services more accessible, equitable, and effective. One that merged critical data and enabled quick, proactive action that would benefit all patients – as well as providers struggling with compliance and cumbersome paperwork.
